What to bring
Photo ID, your insurance card, a list of current medications, and any recent test results or imaging that relate to your visit. All insurance accepted, including Medicaid and Medicare; see patient information for details and forms.
Concierge and virtual care
Members of our concierge service can book directly with their physician. UTI and STI evaluations are also available through the virtual clinic.
